  • Important Information

    The wastewater drainfield area needs to be determined before ANY other plans are made. The following items MUST be completed BEFORE a construction permit application can be reviewed:

    1. Site Assessment must be completed.
    2. All issues stated on Site Assessment must be addressed.
    3. Drinking water sources must be approved by TriCounty Health Department. *Well water will need to be tested prior to health department approval, which takes approximately 4 to 6 weeks.
    4. Site/plot plan must be submitted - this is a plot of the lot showing property lines with dimensions, house/structure location, driveway/parking area, location of onsite wastewater system, location of drinking water source and protection buffer for source. This must be signed by the property owner. 
    5. A certified individual must design the onsite wastewater system (see list of certified indivduals).
    6. TriCounty Health Department requires that a registered or "in-process" installer be the one to install the onsite wastewater system. A homeowner may install their own onsite wastewater system, preferably under the supervision of a registered installer. A liability waiver must also be signed by the property owner. 
    7. Fill out this application completely, including all required attachments. You can save it and come back later if needed.
    8. Submit the following (as prompted) with your application: 
      • Soil Log
      • Perc Test Results
      • Signed attestation to visiting the site and conducting the site assessment in accordance with State and Local wastewater rules.
      • Proof of drinking water (will serve letter, well permit, spring rights)
        • If more than one connection, a shared well/spring agreement will be needed.
      • Detailed System Design
      • Deep Wall Trench photos ( if required)
      • Plot Plan
      • Official floor plan of each building level
      • Current Tax ID Form (if information changed since site assessment was completed)
      • Liability waiver from property owner with homeowner install.
      • Current Tax I.D. Form
      • Fee - $590.00 Standard and Deep Trench systems, $740.00 for alternative system. (Non-refundable and subject to change)

  • Instructions

    Plans must be prepared and signed by a state certified individual and include their certification number. Plans must be prepared in such a manner that the installer can read and follow them in order to install the system properly. Construction shall not commence until the plans have been approved. Uploaded designs must be legible. 

    Materials

    • Schedule, material, and size of pipe to be used
    • Septic tank manufacturer, size of tank, and maximum burial depth of tank
    • Type of gravel to be used (3/4" minimum to 2 1/2" maximum)
    • Type of separation material to be used (straw or fabric)
    • Type of chambers to be used: manufacturer, model, size

    Details

    • Shall be drawn to scale (not to exceed 1 in = 30 ft), indicate scale used
    • Must include risers, clean-outs, and sampling ports as required by rule
    • Location and distance from water supply lines
    • Location of private water source including 100' protection zone
    • Location of 100' protection zone must be indicated on a County Plat showing property line, water source location and 100' in all directions with no source of contamination
    • Locations and distance of any watercourse, lake, pond, or reservoir within 100'
    • Location and distance from curtain drains
    • Location and distance of property lines
    • Location of raodways, driveways, and parking areas
    • Location of easements or right-of-ways affecting property
    • Location and distance from any down slope, cut bank, or top of embankment
    • Location of soil exploration and perc. test holes. Soil exploration holes and percolation tests shall be conducted within 50’ of the proposed drainfield. 
    • Location and size of replacement area
    • Distance from any building foundation
    • Distance between trenches and length of distribution lines
    • Slope of building sewer (must not be less than 1/4" per foot). Includes pipe from building to tank, and tank to drainfield. 
    • Slope of distribution laterals (should be level)
    • If required on site assessment form, relative evaluations MUST be provided. Indicate the original grade and trench bottom for both ends of each distribution lateral.

    • Depth of pipe or chamber inlet
    • Depth of gravel under pipe

    • Depth of backfill
    • Maximum DIG depth of trenches

    Installation Instructions

    • Please provide detailed installation instructions to enable the owner or third party contractor to install the system. Examples: how to enter/exit chamber end caps, orientation of perforated pipe, etc.

  •  On site inspection:

    • Verify Site Assessment Hole through pipe with GPS coordinates
    • Connected to house/ Sewer pipe
    • Check minimum distances from the house foundation to the septic tank 5’.
    • Check Minimum distance from drainfield to foundation standard system 5’ or 20’ for deep wall.
    • Slope of pipe to the tank and tank to drainfield
    • Cleanout (s)
    • Schedule of pipe used
    • Correct size septic tank or larger
    • Tank manufacturer, GPS, and volume
    • Is the tank full
    • Is the tank level
    • Inlet and outlet baffles
    • Tank risers/ Depth of tank
    • Check for zero slope on level headers
    • Inspect D-box (level,water tight,ect.) and verify if they have a riser
    • Check transition from solid to perforated pipe
    • Check that the pipe is in the top of the chamber end cap
    • Count Septic chambers - looking for damaged chambers
    • Check ground fabric or straw
    • Check for end caps
    • Check for maximum trench depth and length
    • If it is a deep trench, measure inside pipe (2 pipes per trench) to verify depth of gravel
    • Verify picture of trench or trenches
    • Check the property line distances if necessary (5’ or 10’ for deep wall)
    • Check right of way and set back distances if necessary (water ways, slopes, and distance to well…etc)
    • Verify correct operation of effluent pump or pressure distribution system

  • By signing this waiver, I acknowledge I have read, understand and agree with the following:

    I assume full responsibility for any damages, injuries, environmental impacts, or regulatory issues that may result from the installation, operation, or maintenance of the wastewater system.


    I release and hold harmless Tricounty Health Department and its employees, agents, and representatives from any and all liability related to the homeowner’s installation or management of the wastewater system.


    I understand that all state and local wastewater rules, permits, and guidelines still apply, and it is my responsibility to ensure compliance.


    I acknowledge that this waiver does not constitute an approval, inspection, or endorsement by any governmental or regulatory agency.


    I understand and agree that any additional inspections of the system due to installation violations will require the additional visit fee. Examples of installation violations include, but are not limited to: tank not being full of water, trench separation, setbacks, pipe slope, max depth, etc.
